1. The Janitor Was Almost A Figment Of JD's Imagination
The pilot episode of Scrubs managed to cram so much into just 23 minutes, introducing all of the show's main characters, as well as several supporting members like Ted and Laverne. One character who was also introduced was the Janitor, portrayed by Neil Flynn.
While fixing a door, the Janitor took something JD said the wrong way, and instantly made himself an enemy. The plan was for the character to appear in no more than one or two episodes, but the dynamic between he and JD, and Neil Flynn in general, was just so funny that Bill Lawrence kept bringing him back.
At one point, since the first episode saw Flynn's character interact with no one else but JD, Lawrence had the idea to reveal further down the line that he had been nothing more than a figment of the doctor's imagination all along.
Thankfully, this wasn't the route the show took, and we got to see the Janitor for eight more seasons, in which he developed the Brain Trust, got married, and even revealed his 'real' name.
