Several of JD and Turk's friends from the early stages of their friendship were alluded to in flashbacks throughout the series. Shinsky fell off a roof when JD threw him a beer, and Caramel Bear may or may not have been killed while trying to pull a prank, but the only one who ever visited them at Sacred Heart was Spence.

Played by Ryan Reynolds, he was exactly how you would imagine a Ryan Reynolds character to be. Spence was funny, charming, extremely likeable, and with a hint of sarcasm that made making fun of his buddies so much better.

He struck a good balance between his serious, professional career, and acting like a kid, screaming down the halls of the hospital to add more drama, and getting drunk in a stranger's hot tub. He was still able to stand up to JD and Turk however, and give them a verbal beat down when they tried to blame him for getting in trouble.

The lasting mark he left on Sacred Heart actually had nothing to do with either of his college buddies, it was that he unwittingly revealed that Jack was in fact Dr. Cox's baby. There was very little guilt shown after it, but somehow that just made it funnier.
