4. Billy
Billy came into Sacred Heart under the pretence of his brother being attacked in a bar, though they weren't actually related, and he was the one who hit him. They'd been out drinking the night before, and while he was waiting for the patient to come around, he ended up chatting with Turk and JD.
He soon showed that he lived his life to the absolute full regardless of what was going on, and implored the two young doctors to do the same. He was the epitome of 'you can sleep when you're dead', and he believed that no harm could come from just trying something new.
He was so damn charismatic and likeable, that even the guy he put in hospital forgave him immediately, and before he left he had both Elliott and Carla falling for him, the latter just because he said her hair was curly. He taught a strong life lesson in his one appearance, something that JD and Turk took to heart going forward.
No one can say whether the character would have been so great had he been portrayed by anyone other than Colin Farrell, but he could have said anything in his Irish accent and audiences would have been compelled. Particularly when it revolves around pink hearts, yellow moons, orange stars, and green clovers.
