One of the longest running errors in the show's production history concerns that of the Beta Gate that is discovered in Antarctica. This is the second entry from the first season episode Solitudes on this list. The plot of this episode revolves around an accident that causes an incoming wormhole to leapfrog over the Alpha Gate, connecting to the previously unknown Beta Gate.

Thankfully, Carter finds a dial home device and sets about digging it out of the ice. Once this is completed, she works out the address of Earth, based on the unique point of origin symbol. As established in the Stargate movie, carrying over into the series, a Gate address requires seven symbols to connect. Six symbols determine the Gate's location and the seventh symbol denotes the starting point.

So, this raises two issues. First, the point of origin for this Gate should be the exact same as on the Alpha Gate - a pyramid, under a star. Instead, it is a star over a straight line.

The second issue that this raises...is that after this episode, it is never spoken of again. The Beta Gate substituted for the Alpha Gate for several years, in fact. In all that time, it was simply never mentioned. Obviously, this was done to streamline the show - but it is a bit of a gaff all the same!
