The one entry in this list that didn't take years to come out, but over a decade since release it's clear a fraction of those who played Heavy Rain have ever been shown its deleted scenes.

As a result, the base game left a gaping plot hole that everyone still talks about to this day:

How or why was Ethan suffering from blackouts, and why does he wake up standing in the middle of a road, holding a tiny origami model, just like that of the serial killer everyone's hunting down?

The reason, as we'd find out in Heavy Rain's barely available deleted scenes, was that after Ethan was hit by a car attempting to rescue his son Jason, this formed a "psychic link" with the Origami Killer. Ethan was shown descending into a flooded dreamworld that represented the killer's mind whenever the latter murdered someone, folding an origami model himself while the killer did their bidding.

Quantic Dream removed the scenes as it was "too much of a supernatural element", but it also left an insane plot hole only remotely addressed if you hunt down these scenes yourself.
