All the way back in Season 1, Episode 2. Sentenced to execution in place of Arya's wolf Nymeria, Lady is taken away to be killed by Ned Stark himself, rather than the king's executioner. We see Ned raise his sword and bring it down on something that whimpers off-screen.

Could She Still Be Alive?

It feels like the idea of the fate of the direwolves as tied to the fate of the Stark family does kind of require some of both to make it to the final episodes. Only Jon's albino runt of the litter Ghost has been a regular player on the whole run of the show, though.

Arya's Nymeria, however, disappeared in the same episode as Lady before finally returning as a giant wolf at the head of her own pack in the last season. So, Game of Thrones is perfectly capable of bringing something back after all that time.

With the other direwolves, like Robb's Grey Wind and Rickon's Shaggydog, the show did not stint on its usual graphic gore (showing off both their severed heads), and the wolves' deaths were closely tied to that of their masters. So, the death of Sansa's Lady feels quite different.

Could the decision to show Ned taking on the duty himself and then not actually depict the beheading suggest that the Warden of the North spared his daughter's faithful companion and executed a substitute? Probably not, but the return of Nymeria does suggest that there's more to come from the direwolf-Stark connection.
