5. Patrick Kluivert (1996) - Ajax 1-1 Juventus A.E.T. (The Italians Won 4-2 On Penalties)

Patrick Kluivert had been the hero for Ajax off the bench the previous season but he was unable to deliver again in 1996 as Louis van Gaal's team came agonisingly short of defending the Champions League title. Fabrizio Ravanelli had put Juventus ahead before Jari Litmanen equalised for the Dutch side at the Stadio Olimpico in Rome. For the second final in succession Kluivert was called upon off the bench, on this occasion at half-time, but he was unable to score - and he didn't even take a spot-kick as Ajax succumbed to a 4-2 defeat on penalties.
