11. Patrick Kluivert (1995) - Ajax 1-0 AC Milan
When Patrick Kluivert arrived at Newcastle United in 2004 he was already beginning to look like a player who was struggling to reach the marvellous heights he had previously - but back in his day he was a brilliant player. Although he didn't start the 1995 Champions League final for Ajax against AC Milan he certainly made a massive impact off the bench - netting the winner with five minutes to go. That goal proved enough for Louis van Gaal's men to lift the Champions League trophy in Vienna, and Kluivert will always be remembered as the man who helped Ajax claim their most-recent European Cup triumph.
